Our philosophy on patient privacy

We recognize the importance of patient privacy. As such, it is our policy to treat all medical information as personal and confidential; we are committed to protecting your privacy rights. We will create a record of care and services you receive at our practice.

Permitted Uses and Disclosure: the privacy standard identifies certain permissible uses and disclosures, without the need to obtain certain written consent or authorization form, the patient. The following are permissible uses and disclosures:

PATIENT'S RIGHTS

Patients must be able to see and obtain copies of their records and to request changes.
